Responsibilities
- Provide direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate medical documentation in electronic health record (EHR) systems like Epic or Cerner
- Support patients with specialized needs including Alzheimer's care, dementia care, memory care, and behavioral health challenges
- Assist with medical procedures such as catheter care, medication administration under supervision, phlebotomy, and aseptic techniques to maintain infection control standards
- Collaborate with interdisciplinary teams in various settings such as hospitals (including ICU and Level I Trauma Centers), nursing homes, assisted living facilities, outpatient clinics, and home & community care environments
- Ensure compliance with HIPAA regulations by safeguarding patient confidentiality during all interactions and documentation processes
- Use medical terminology accurately while documenting patient information in electronic systems like eClinicalWorks or EMR/EHR platforms
Skills
- Proven experience working in diverse healthcare environments including long-term care, acute care, hospital medicine, or outpatient clinics
- Critical care experience such as ICU or Level II trauma center familiarity highly valued
- Knowledge of advanced patient care techniques including dementia and Alzheimer's disease management, hospice care, infectious disease protocols, and behavioral health support
- Proficiency with medical documentation systems like Cerner or Epic; familiarity with EMR/EHR systems is essential
- Strong understanding of physiology and anatomy relevant to patient monitoring and treatment procedures
- Ability to perform basic life support (BLS), vital signs measurement, catheter care, and medication administration accurately
- Compassionate caregiving skills with experience working with individuals with disabilities or developmental challenges; experience in pediatrics or geriatrics is advantageous
- Excellent communication skills to effectively collaborate within multidisciplinary teams and provide exceptional patient service
