Job Summary - Hiring for Home Health and Hospice Visits

Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) systems
- Support patient care in home care environments
- Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or behavioral health needs including memory care and dementia-specific activities
- Assist with medical procedures such as catheter care, phlebotomy preparation, and basic life support during emergencies or urgent care situations
- Maintain accurate medical documentation and records while respecting patient confidentiality in accordance with HIPAA regulations
- Collaborate with interdisciplinary teams in post-acute settings to deliver comprehensive patient service
- Support infection control practices by adhering to aseptic techniques and safety protocols in all patient interactions
Skills
- Proven experience working with individuals with disabilities, including developmental disabilities and behavioral health conditions
- Knowledge of medical terminology, physiology, anatomy, and patient care procedures across various specialties such as geriatrics, pediatrics, emergency medicine, hospice care, and hospital medicine
- Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation
- Experience in high-acuity environments like Level I Trauma Centers or Level II Trauma Centers is a plus
- Ability to perform catheter care, medication administration, vital signs monitoring, and basic life support confidently
- Strong caregiving skills with a focus on patient service excellence in inpatient or outpatient settings
- Working knowledge of infection control standards including aseptic technique and infectious disease care protocols
- Excellent communication skills to collaborate effectively within multidisciplinary teams in fast-paced environments such as ICU or urgent care clinics
- Compassionate approach to caring for seniors in long-term care facilities or assisted living communities; experience with memory care is highly valued
- Ability to operate medical equipment such as Hoyer lifts safely and efficiently while maintaining patient dignity
