Chronic Care Manager

We have an opening for Chronic Care Manager, LPN, in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. You must be an LPN to qualify for this job. We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.

A chronic care manager does a combination of administrative work as well as case management. Job duties include but are not limited to; order processing, phone call communications with family and other providers, prior authorizations, scheduling, med reconciliation, case management. You are the direct liaison between our company and the facilities we service. You will be working directly with NPs that we hire through the company that provides primary care and psych services to our patients.

Our focus is in assisted living communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical needs. Starting pay is $55k, but with our incentive pay structure, our CCMs average around $78k/year.

Being a mobile practice we go out to Assisted Living communities to provide services. We go to 1 community each day. Our current need is in Carmel and Noblesville. We travel to 1 community per day, but all of our communities weekly. Mileage is not reimbursed.

Due to the unique nature of the position, we do require you to participate in a 2 hour shadow with one of our local providers to ensure the position is a good fit for both you and the company. We thank you in advance for your willingness to participate.
