Position Overview

The Client Care Coordinator is a vital role within our non-medical home care agency, serving as the primary connection between families in need and our care services. This position requires a compassionate, confident, and sales-oriented professional who can meet with families in their homes or healthcare settings, explain services clearly, build trust, and convert inquiries into active clients.

Home Care Sales / Marketing Representative

Key Responsibilities

Client Intake & In-Home Visits

  • Meet with prospective clients and family members in their homes, hospitals, or rehabilitation facilities
  • Present non-medical home care services in a clear, professional, and compassionate manner
  • Serve as a trusted resource for families during the decision-making process
  • Discuss services, schedules, and pricing confidently and ethically
  • Convert inquiries and referrals into active clients
  • Follow up consistently with prospects to close services
  • Meet or exceed monthly sales and intake goals
  • Obtain signed service agreements and initiate care services
  • Follow up after visits to answer questions and close pending cases
  • Build and maintain referral relationships with hospitals, case managers, social workers, rehab centers, and community organizations
  • Conduct outreach visits, presentations, and networking activities
  • Represent the agency at community events, health fairs, and professional meetings
  • Identify new marketing and referral opportunities within the community

Care Coordination & Communication

  • Communicate effectively with caregivers, Registered Nurse (RN), and management
  • Maintain ongoing relationships with clients and families to ensure satisfaction
  • Address concerns professionally and ensure a positive client experience

Marketing, Networking & Outreach

  • Build strong relationships with referral sources such as hospitals, case managers, social workers, and community organizations.
  • Participate in community events, outreach activities, and networking opportunities
  • Represent the agency professionally in the community
  • Identify opportunities to increase referrals and client engagement

Relationship Management

  • Build trust and long-term relationships with clients, families, and referral partners
  • Serve as a knowledgeable resource regarding home care services
  • Communicate regularly with internal staff to ensure smooth service start-ups

Required Qualifications

  • Fluent in English (verbal and written) – REQUIRED
  • Strong verbal, written, and interpersonal communication skills
  • Proven ability to build trust and rapport with clients and families
  • Sales experience with a consultative, relationship-based approach
  • Comfortable conducting in-home visits and handling sensitive conversations
  • Strong phone skills, including follow-ups and outbound calls
  • Excellent organizational and time-management skills
  • Ability to work independently and meet intake/sales goals
  • Valid driver’s license and reliable transportation
  • Basic computer skills (email, scheduling software, Microsoft Office or Google Workspace)

Preferred Qualifications

  • Experience in non-medical home care, healthcare, senior services, hospice, or related fields
  • Background in sales, intake coordination, client relations, or community outreach
  • Knowledge of local community resources and referral networks
  • Experience working with seniors, individuals with disabilities, or chronic conditions

Pay: $25.00 - $30.00 per hour
