POSITION SUMMARY:
This Clinical Care Coordinator will blend on-site employee health care delivery with remote prescribing support and provider mailbox management for part-time or off-board clinicians. The successful candidate will provide high-quality, patient-centered care in person, support prescribing workflows remotely, and maintain effective provider communication and administrative follow-up to ensure continuity of care across the organization.
This is a part-time hybrid position (total of 3 days per week): 1 day in-person at one of the health centers for purposes of employee health services and 2 days remote for purposes of prescribing support and mailbox/provider services. Operational needs may dictate occasional additional hours for scheduling or urgent care coverage, with appropriate notice whenever possible.
DUTIES AND RESPONSIBILITIES:
Essential Functions:
In-Person Employee Health Services (1 day/week) – Manhattan Location

Deliver comprehensive acute and preventive primary care to employees in the on-site employee health clinic.

Perform clinical assessments, treatment planning, vaccinations, minor procedures as appropriate, and health risk screenings.

Manage immunization records, health surveillance, and OSHA assessments as applicable.

Maintain accurate and complete medical records in the electronic health record (EHR) system.
Remote Prescribing Support (2 days/week)

Review prescription requests for all clinicians, validate clinical appropriateness, and issue refills as authorized within scope and institutional guidelines.

Ensure timely processing of prescription requests while adhering to regulatory requirements, formulary constraints, and safety protocols.

Communicate with prescribers regarding clarifications, contraindications, or needed documentation.

Monitor medication safety, including reconciliation and adverse event reporting when necessary.

Document all remote prescribing activity in the EHR and relevant systems.
Provider Support & Mailbox Management

Manage provider inbox in the EHR for part-time or off-board clinicians.

Triage and respond to inquiries related to credentialing, scheduling, patient care coordination, and administrative concerns.
Collaboration and Compliance

Collaborate with Occupational Health and Human Resources to optimize workflows for Employee Health Services.

Collaborate with the Director of Advanced Practice Nursing to ensure compliance with all applicable laws and regulations regarding prescriber requirements and HIPAA standards.

Participate in quality improvement initiatives and contribute to patient satisfaction objectives.

Documentation & Reporting

Maintain thorough, accurate documentation in the EHR and billing systems.

Generate regular reports on clinic activity, remote prescriptions, and mailbox/support metrics for leadership.
EDUCATION AND EXPERIENCE

Minimum of a Master’s degree in Nursing required.

NYS Licensed Nurse Practitioner (Family Medicine; board-certified (or eligible) in Family Medicine, required

Minimum of three (3) years of clinical experience in primary care or family medicine; prior experience in employee health or occupational health is preferred.

Current prescriptive authority and must be comfortable with remote prescribing workflows; familiarity with electronic prescribing systems (ePrescribing).[MJ1]
