Long Hollow Family Practice is a family-oriented, privately owned primary care practice dedicated to providing exceptional patient care while fostering a supportive and collaborative work environment for our staff. We are seeking a compassionate, organized, and motivated Primary Care Nurse / Clinical Coordinator to join our growing team.
Position Summary
The Primary Care Nurse / Clinical Coordinator is responsible for providing safe, efficient, and patient-centered care in a busy outpatient setting. This role combines hands-on clinical nursing responsibilities with administrative care coordination duties to help ensure patients receive timely, high-quality care and access to needed services.
Clinical Responsibilities
- Obtain and document vital signs, medical histories, allergies, medications, and chief complaints
- Prepare patients for examinations and procedures
- Assist providers during patient evaluations and treatments
- Perform patient triage and communicate urgent concerns appropriately
- Administer medications and treatments as ordered
- Collect blood specimens through venipuncture and capillary collection
- Perform in-office diagnostic testing and specimen processing
- Provide patient education regarding medications, treatment plans, and follow-up care
- Maintain accurate and timely EMR documentation
- Follow OSHA, HIPAA, infection control, and clinic safety protocols
Front Desk & Care Coordination Responsibilities
A significant portion of this role will focus on:
- UnitedHealthcare referral management
- Prior authorization submissions and follow-up
- Referral coordination and specialist scheduling
- Medication adherence and medication non-adherence outreach programs
- Tracking referral statuses and ensuring completion of patient care plans
- Communicating with insurance companies, pharmacies, specialists, and patients
- Coordinating follow-up appointments and patient care needs
Qualifications
- Active LPN or RN license preferably
- Previous primary care, family practice, or outpatient experience preferred
- Experience with referrals, prior authorizations, and insurance coordination preferred
- Strong knowledge of EMR systems and clinical documentation
- Excellent communication, organizational, and multitasking skills
- Ability to work independently and as part of a team
- Phlebotomy experience preferred or willingness to be trained our office way

Pay: $15.00 - $19.50 per hour
