Clinical Informatics & Data Strategy Pharmacist

Overview

Join our specialty pharmacy team as a Clinical Pharmacist – Specialty & Compliance, where you will provide patient-centered clinical care while supporting regulatory excellence and operational integrity. This role focuses on specialty medication management, therapeutic monitoring, accreditation compliance, and accurate documentation within a multi-state pharmacy environment.

You will collaborate with prescribers, nurses, regulatory leadership, and operations teams to ensure optimal patient outcomes and alignment with URAC, ACHC, and state regulatory standards. The position also offers exposure to pharmacy analytics and internal reporting initiatives.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans aligned with specialty pharmacy accreditation standards.
  • Provide disease-state education, medication counseling, adherence support, and self-management guidance.
  • Perform therapeutic monitoring, including lab review, adverse event assessment, medication reconciliation, and coordination with prescribers to ensure safe and effective therapy.
  • Manage refill authorizations and maintain continuity of care to prevent therapy interruptions.
  • Document all clinical activities accurately and contemporaneously in accordance with URAC, ACHC, HIPAA, and regulatory requirements.
  • Identify and report medication errors or adverse drug reactions per pharmacy policy.
  • Participate in Quality Management Committee initiatives and support accreditation audits and regulatory reviews.
  • Assist with internal compliance monitoring related to specialty and 340B operations.
  • Contribute clinical insight to operational reporting and participate in payer-related discussions as needed (non-lead role).
  • Obtain additional pharmacist licensure in states requiring non-resident pharmacy coverage as needed.

Qualifications

  • Current, unrestricted Pharmacist license issued by the New York Board of Pharmacy.
  • Graduate of an accredited college of pharmacy; PharmD preferred.
  • Minimum of 3 years of experience as a practicing pharmacist; specialty or ambulatory care experience preferred.
  • Working knowledge of specialty pharmacy accreditation standards (URAC and ACHC preferred).
  • Strong organizational skills with attention to detail and regulatory compliance.
  • Proficiency in Excel or similar reporting tools preferred.
  • Excellent communication and collaboration skills.

Job Type: Full-time

Pay: $68.00 - $72.50 per hour

Expected hours: 40 per week
