Job Title: Clinical Liaison – RN or LPN | Skilled Nursing Facilities
Location: Antigo, WI & Laona, WI (Wausau Market)

Champion Care is seeking a passionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Clinical Liaison for our skilled nursing facilities in Antigo and Laona, Wisconsin. This is a key role focused on driving census growth, managing hospital relationships, and coordinating smooth transitions of care for both facilities.

Key Responsibilities:

• Serve as the primary liaison between local hospitals and Eastview Health and Rehabilitation Center and Nu Roc Health and Rehabilitation Center.

• Review clinical information and determine eligibility for admission to our facilities.

• Develop and execute strategic marketing plans to increase visibility and drive census.

• Build and maintain strong relationships with discharge planners, case managers, and hospital staff.

• Collaborate with internal clinical and admissions teams to ensure timely and appropriate admissions.

• Monitor market trends and identify new referral sources.

• Represent Champion Care at networking and community events.

Qualifications:

• Active RN or LPN license in the state of Wisconsin (required)

• Experience in skilled nursing, healthcare marketing, case management, or admissions preferred

• Strong clinical assessment and decision-making skills

• Excellent communication and relationship-building abilities

• Ability to travel regularly between Antigo and Laona
