The Clinical Navigator - Sales plays a vital role in bridging the gap between acute care and post-acute services by identifying patients who are suitable for home health and hospice services. This role serves as the clinical liaison between hospital case managers, physicians, patients, and families to ensure a seamless transition of care. The Clinical Navigator conducts bedside assessments, facilitates referrals, and supports business development efforts through the application of clinical expertise and relationship-building.
Key Responsibilities:

Perform bedside clinical assessments of hospitalized patients to determine eligibility and appropriateness for home health or hospice services.

Collaborate closely with hospital discharge planners, case managers, physicians, and nursing staff to identify patients in need of post-acute care.

Educate patients and families about available services, care options, and what to expect upon discharge.

Coordinate and facilitate timely and appropriate referrals to home health and/or hospice care.

Maintain strong knowledge of Medicare, Medicaid, and commercial insurance guidelines regarding eligibility for services.

Provide accurate and timely documentation of all assessments, interactions, and referral activities in accordance with company and regulatory standards.

Act as a clinical resource to the sales and admissions teams, supporting business development with professional expertise.

Follow up on hospitalized patients to track progression and reassess care needs as appropriate.

Develop and maintain positive relationships with hospital staff and other healthcare professionals to promote service lines and generate referrals.

Participate in marketing activities, community outreach, and educational events as needed.

Current, active RN or LVN/LPN license in the state of employment (RN preferred).

Minimum of 2 years clinical experience in acute care, home health, or hospice.

Strong understanding of home health and hospice eligibility criteria, regulations, and services.

Experience with sales, marketing, or liaison roles in a healthcare setting preferred.

Excellent interpersonal, communication, and customer service skills.

Ability to work independently, manage time effectively, and adapt to a fast-paced hospital environment.

Reliable transportation and willingness to travel to local hospitals and facilities.
Work Environment:
