Clinical Nurse Navigator - Oncology

• Works in clinic with some remote hours with Oncologists to coordinate care for Oncology patients
• Orders diagnostic testing in coordination with Oncologists
• Outgoing referrals to specialists and community services
• Schedules follow up appointments with Oncologists
• Documentation in Epic
• Occasional on-site meetings with Oncology Nursing Manager and/or Oncologists
• Converses with patients to help navigate them on their cancer journey
• Identifies and anticipates patient and/or provider needs and develops evidence-based quality programs to meet those needs. Ensures delivery of quality services for patients.
• Maintains a thorough knowledge and understanding of evidence based/best practice.
• Develop Oncology Nurse Navigation role in coordination with Oncology Nursing Manager
• Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
• Protect patient confidentiality by promoting professional staff communications.

Tactile perception is also important for assessing physical characteristics such as temperature, size, shape, or texture during patient care. Motor coordination is required for reaching, grasping, handling, and performing fine motor tasks necessary for the use of medical equipment and procedures. The position also involves occasional stooping, kneeling, crouching, or crawling when assisting patients or accessing equipment. Walking and standing are typically intermittent throughout the shift. May need to sit for long periods.
