QUALIFICATION:
- Pharm (Pharmacology / Clinical Pharmacy) / Doctor of Pharmacy (Pharm. D)
EXPERIENCE:
JOB RESPONSIBILITIES:
Patient Care & Clinical Services
- Review and evaluate medication orders for accuracy, appropriateness, and potential interactions.
- Monitor patient responses to prescribed drug therapies and make evidence-based recommendations.
- Participate in ward rounds and clinical case discussions with medical teams.
- Counsel patients and caregivers on the use of medications, side effects, and adherence.
- Support medication reconciliation during patient admission, transfer, and discharge.
Drug Therapy Management
- Monitor therapeutic drug levels and adjust dosages accordingly.
- Implement pharmacovigilance protocols and report adverse drug reactions (ADRs).
- Recommend appropriate drug alternatives based on patient condition, cost, and formulary.
Collaboration & Communication
- Serve as a resource for physicians and nurses regarding medication information.
- Communicate any changes in medication regimens clearly to the healthcare team.
- Coordinate with the pharmacy department for timely medication supply and formulary updates.
Medication Safety & Compliance
- Ensure compliance with national guidelines, hospital policies, and regulatory requirements.
- Participate in drug audits, inventory checks, and quality improvement initiatives.
- Promote antimicrobial stewardship and monitor antibiotic usage.
Documentation & Reporting
- Maintain accurate records of interventions, ADRs, and therapeutic outcomes.
- Contribute to clinical documentation in patient medical records.
- Assist in compiling data for hospital committees (e.g., Pharmacy & Therapeutics Committee).
Training & Education
- Educate interns, pharmacy students, and hospital staff on drug-related topics.
- Participate in continuing education and stay updated on recent advances in clinical pharmacy.
- Any other work/responsibility assigned by the HOD/Management in the course of business/work whether within the same unit/dept or any other unit/dept from time to time.
