1. Clinical Care & Medication Management
- Review and verify medication orders for accuracy, safety, and appropriateness.
- Perform clinical interventions such as dose adjustments, drug therapy optimization, and therapeutic monitoring.
- Provide pharmacokinetic consultations (e.g., vancomycin, aminoglycosides).
- Identify, prevent, and resolve medication-related problems.
2. Patient Counseling & Education
- Counsel patients on proper medication use, adherence, side effects, and lifestyle considerations.
- Provide discharge medication education to ensure understanding and safe continuation of therapy.
3. Collaboration & Interdisciplinary Support
- Participate in daily rounds with physicians, nurses, and other healthcare professionals.
- Offer evidence-based drug information and recommendations to support clinical decision-making.
- Assist in the development of patient care protocols, guidelines, and formulary management.
4. Safety & Quality Assurance
- Monitor for adverse drug events (ADEs) and document interventions.
- Ensure compliance with hospital policies, regulatory requirements, and best practice standards.
- Participate in quality improvement initiatives and medication-use evaluations.
5. Operational Support
- Oversee medication preparation, dispensing, and distribution processes as needed.
- Support pharmacy technicians and provide guidance for complex medication-related tasks.
6. Education
- Doctor of Pharmacy (PharmD) degree or Bachelor's degree in Pharmacy (BPharm) with equivalent clinical experience required.
- PGY-1 or PGY-2 residency preferred (optional depending on organization).
Licensure
- Valid and active pharmacist license in the state/country of practice.
- Board certification (e.g., BCPS, BCOP, BCACP) preferred but not mandatory.
Experience
- Clinical pharmacy experience in a hospital or ambulatory care setting preferred.
Skills & Competencies
