Job Description
Position Summary
Provides clinical pharmacy services on an as-needed, per diem basis to ensure safe, accurate, and timely medication use. Performs independent clinical review and operational oversight during assigned shifts, maintaining continuity of care and supporting departmental workflow.
Per Diem Role Expectations

The schedule is created six weeks in advance. Per diem staff are asked to submit at least six shifts of availability (not a commitment to work all six) to support scheduling flexibility.

Flexibility to work variable shifts, including weekends, evenings, and holidays, based on departmental needs.

Practices independently with minimal supervision while ensuring safe and efficient pharmacy operations

Maintains competency to function across all assigned practice areas
Key Responsibilities
Clinical Review & Order Verification

Review and verify all medication orders for safety and appropriateness (drug selection, dose, route, allergies, interactions, duplication, labs, diagnosis, age)

Collaborate with prescribers as needed; document interventions per policy

Accurately enter and verify orders in the pharmacy information system, including administration instructions
Shift-Based Operations

Oversee pharmacy operations during assigned shifts, ensuring continuity of medication distribution

Manage medication distribution including controlled substances, IV medications, compounded and repackaged products, and investigational drugs

Utilize pharmacy technology systems (Omnicell) to support safe and efficient workflow
Supervision & Collaboration

Supervise pharmacy technicians and interns on duty; provide guidance to ensure safe medication practices

Collaborate with nurses, prescribers, and other healthcare professionals to support patient/resident care

Communicate clearly with incoming staff to ensure appropriate handoff of outstanding issues
Quality, Compliance & Professional Practice

Adhere to all departmental and organizational policies, procedures, and customer service standards

Maintain required competencies, licensure, and continuing education

Participate in quality improvement initiatives as appropriate for per diem staff

Support a culture of safety, professionalism, and patient-centered care

Qualifications
Education:
-
PharmD or BS in Pharmacy from an accredited institution
Licensure
-
Active Massachusetts Pharmacist License (required)
