Key Responsibilities:
- Patient Care & Monitoring:
- Participate in ward rounds, reviewing patient cases and medication plans.
- Monitor patient progress and drug effects, intervening to resolve issues.
- Manage medication therapy for specific patient groups (e.g., cardiology).
- Medication Management:
- Review and verify prescriptions for appropriateness, dosage, and interactions.
- Perform medication reconciliation (comparing home meds to hospital meds).
- Ensure proper drug preparation, storage, administration, and compliance with policies.
- Collaboration & Communication:
- Consult with physicians, nurses, and other healthcare professionals on drug therapy.
- Educate patients and caregivers on medications, side effects, and adherence.
- Provide drug information and support to the healthcare team.
- Safety & Quality:
- Implement medication safety protocols, including managing crash carts and controlled substances.
- Contribute to policy development and quality improvement initiatives.
- Handle specialized areas like unlicensed medicines or clinical trials.
- Operational Duties:
- Manage ward pharmacy stock and security.
- Supervise junior staff, students, and technicians.
