Pay Range: $20.19 - $31.80 per hour

Summary:

The clinical resource coordinator 1 (CRC 1) position applies a collaborative approach in working with pharmacists, patients, medical providers, payers and internal/external agencies to provide case management support services related to individual patients’ medication needs. CRC 1 activities and processes include referral screening, patient interviews, completion of medication access processes (e.g., prior authorizations, financial assistance programs), follow-up calls, and working within prescription processing software. CRC 1 services may be coordinated centrally through the pharmacy department or assigned by unit, physician practice, or service line. Responsibilities for specific liaison roles may vary depending upon assignment. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures

Required Job Qualifications


  • Valid State of Illinois Pharmacy Technician license required
  • High School diploma and minimum 1 to 2 years’ experience required.
  • Demonstrated knowledge and/or experience with hospital -related service functions such as: patient interviews, discharge planning/social service basic assessment and referral processes, community resources, finance and payer communication processes and information systems.
  • Maintains CE hours required to retain CPhT certification. Obtain certification within 1 year of starting if not certified prior to starting.

Responsibilities

  • In conjunction with other pharmacy team members, CRC 1s manage patient caseloads, serve as a resource, and set priorities to complete assigned medication access tasks.
  • Facilitate collaboration between pharmacy teams and/or other departments to prioritize tasks, ensuring patient support needs are met.
  • Model ICARE values and actively supports teamwork, customer service and employee engagement. Flexes schedule to meet department needs.
  • Facilitate effective, efficient communication and problem solving with internal and external customers. Use clinical software to document activities and track performance outcomes.
  • Seek continuing education opportunities, complete required in-services and maintain professional growth by attending various department, institutional and external meetings, seminars and workshops.
  • Apply performance improvement strategies to promote continuous process improvement.
  • Perform activities in support of patient flow that may include patient placement activities.
  • Assume other related responsibilities as assigned.
