Summary

We are seeking a compassionate and dedicated Social Worker to join our team at Home Dialysis Therapies. In this role, you will play a vital part in supporting patients through their treatment journey, helping them navigate the challenges of end-stage renal disease while promoting their overall well-being.

Responsibilities

• Provide psychosocial services to patients undergoing home dialysis.
• Conduct assessments to identify patient needs and develop individualized care plans.
• Facilitate group therapy sessions and provide clinical counseling as needed.
• Assists with vocational maintenance/rehabilitation
• Promotes adherence to treatment regimen to optimize outcomes, works with patients and their support system to improve quality of life
• Supports transplant candidacy and helps patients navigate the process
• Collaborates with administrative staff to address insurance issues, assists with access to state/federal/public/private financial resources,
• Support patients and their families in crisis situations, offering guidance and resources.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Educate patients about their treatment options and support them in making informed decisions.

Requirements

• Master's degree in Social Work (MSW) from an accredited program.
• Valid state licensure as a Social Worker (LCSW or equivalent).
• Experience in case management or clinical counseling preferred.
• Demonstrated knowledge of government and private insurance programs
• Dialysis experience, particularly with the home dialysis population, preferred
• Minimum of 2 years’ experience as a social worker in a healthcare setting
• Strong communication skills and ability to work collaboratively with healthcare teams.
• Compassionate approach with a focus on patient-centered care.

Job Type: Part-time

Pay: $36.00 - $55.00 per hour
