JOB SUMMARY

Wake Forest University School of Medicine, Department of Implementation Science presents an exciting opportunity for nurses passionate about delivering telephonic palliative care management. In this role, the successful candidate will provide telephonic palliative care to patients living with Alzheimer’s Disease and other related dementia after discharge from the Emergency Department. Nurse-led telephonic case management will be conducted for clinical sites across Atrium Health regions in North Carolina. Under the direction of the investigator for Nurse Led Telephonic Care, the Clinical Research Nurse II is responsible for completing assigned training, attending weekly team meetings, and protocol adherence. Clinical responsibilities include transitional care delivery, serious illness communication and establishing goals of care, symptom assessment and management, collaborating with key healthcare providers to coordinate care to meet patient and family needs, and caregiver support. The successful candidate will receive clinical training on telephonic administered care to ensure appropriate patient-caregiver-clinician communication and effective palliative care delivery.

EDUCATION/EXPERIENCE

A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associate Degree in Nursing requires a minimum of one year experience. Hospice and palliative nursing certification greatly preferred (e.g. CHPN, ACHPN). Experience with case management, telephonically administered care or telemedicine preferred. Hours will be flexible to the number of patients referred to the program on a weekly basis and calls will need to engage with patient and caregiver availability.

LICENSURE, CERTIFICATION, and/or REGISTRATION

Licensure as a Registered Nurse (RN) in the State of North Carolina required. Must complete the CITI certification for Human Subject Research if not already completed and any additional research-related training requirements.

ESSENTIAL FUNCTIONS

  • Works under the direction/supervision of the Study Investigators.
  • Knowledge and understanding of policies, procedures, and regulations governing human subject's research incorporates them in the conduct of research and care of participants.
  • Conduct thorough initial assessments during telephonic consultations, considering the specific needs and conditions of Persons Living with Dementia and their care partners, following specific program protocols.
  • Develop individualized care plans based on the assessments, prioritizing issues identified in collaboration with the person living with dementia and care partner/caregiver(s) in accordance with training and protocols.
  • Implements interventions identified in the plan of care.
  • Collects patient healthcare data and systematically maintains accurate and detailed documentation of all telephonic consultations, assessments, care plans, and interventions.
  • Perform medication reconciliations, ensuring accurate and up-to-date medication lists, and address any concerns or discrepancies.
  • Evaluate patient progress toward attainment of expected outcomes over 6-month periods.
  • Educates the patient and care partner/caregiver(s) to facilitate and promote healthy adjustment to new or changed health patterns.
  • Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.
  • Schedule and conduct follow-up meetings after interdisciplinary team discussions, reviewing care plans and making necessary adjustments.

SKILLS/QUALIFICATIONS

  • Good oral, written, listening, and interpersonal and communication skills
  • Ability to provide indirect patient care
  • EPIC/WakeOne proficiency to include appropriate documentation of research notes
  • Basic computer skills required

WORK ENVIRONMENT

  • Requires reliable phone service to deliver telephonic care
  • Professionalism and customer service skills required at all times
  • Occasionally subject to irregular hours
