Cluster Care HHA/PCA – $25/hr – Bronx/Manhattan/Brooklyn

Overview
Join our dedicated healthcare team as a Cluster Care Home Health Aide (HHA) / Personal Care Assistant (PCA), providing compassionate, high-quality care to individuals in their homes across Manhattan, Brooklyn, Bronx and surrounding areas. Responsibilities

  • Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals
  • Assist with medication administration following prescribed protocols and maintaining accurate medical documentation
  • Support patients with specialized needs such as dementia care, Alzheimer’s care, memory care, or hospice support
  • Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
  • Maintain detailed records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
  • Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment
  • Collaborate with nursing staff and other healthcare providers to coordinate patient care plans and document progress accurately

Requirements

  • Valid Home Health Aide (HHA) or Personal Care Assistant (PCA) certification required
  • Prior experience in home health, assisted living, nursing homes, or long-term care settings preferred
  • Experience working with individuals with disabilities or developmental disabilities highly desirable
  • Ability to assist with complex medical tasks such as catheter care, medication administration, and hospice support
  • Comfort using medical equipment like Hoyer lifts and performing vital signs assessments
  • Strong communication skills to effectively observe patient conditions and collaborate with healthcare teams
