Responsibilities
- Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts.
- Monitor and record vital signs, observe patient conditions, and report changes promptly to nursing staff.
- Support patients with specialized needs including dementia care, Alzheimer's care, memory care, and behavioral health management.
- Maintain detailed medical documentation within Electronic Health Record (EHR) systems to ensure compliance with HIPAA regulations.
- Use medical terminology accurately while documenting patient records and vital signs to support continuity of care.
Experience
- Prior experience working with home health and long-term care facilities such as nursing homes or assisted living communities is highly valued.
- Knowledge of physiology, anatomy, infection control practices (including aseptic technique), and medication administration procedures.
- Background working with individuals with disabilities or developmental delays, including behavioral health considerations.
- Certification in Basic Life Support (BLS) is required; additional certifications like Phlebotomy or DME experience are advantageous.
