CO Mental Health Case Manager

JOB SUMMARY

Reporting directly to the Medical Lieutenant, responsible for coordinating with the local mental health authority and being the prime interface providing person-centered case management of adults who are diagnosed with mental health disorders currently in custody of the county jail. Coordinates with the County Attorney’s office in the pursuance of involuntary medication hearings for inmates and facilitates the dissemination for pertinent medical records in dealing with those court hearings.

EXAMPLES OF WORK PERFORMED

  • Oversees multiple medical orders for medications of inmates on compelled medications
  • Coordinates with multiple agencies in the implementation of care
  • Manages and maintains medical database associated with compelled medications
  • Coordinates with outside forensic psychologists regarding competency and Not Guilty by Reason of Insanity (NGRI) evaluations
  • Coordinates and interacts with jail psychologist in the process of forced medication applications and ensures the documentation is presented to the County Attorney’s office
  • Coordinates and interfaces with mental health attorneys in the care of their clients in the county jail
  • Coordinates with the Crisis Intervention Team and the Local Mental Health Authority in the process of dismiss and transfer of inmates to outside mental health facilities
  • Communicates with the Magistrate and District Judges in the care of psychiatric patients in custody of the county jail
  • Participates in the quarterly mental health round table and the Jail-Based Competency Restoration Program (JBCR) monthly meetings
  • Reviews, tracks, and verifies all inmates on the 46B and 16:22 of the Code of Criminal Procedures
  • Assists in annual budget process by collaborating with department leadership regarding medication and needs for psychiatric patients
  • Performs purchasing coordination for the department, to include generating purchase requisitions, processing invoices, management of relationships with suppliers, and coordination with chain of command
  • Initiates and cancels medication orders into the Electronic Medical Record as needed
  • Reviews, assesses, and documents information, and coordinates the implementation of treatment plans to provide better outcomes
  • Generates Texas Uniform Health Status documentation for transferring/departing inmates as needed
  • Attends all meetings and trainings, as required
  • Performs special duties as assigned

PHYSICAL DEMANDS
  • Position involves prolonged sitting at a workstation, visual examination of documents, reaching, walking, twisting, bending, standing and occasional lifting/carrying
  • Position involves occasional lifting and carrying of up to fifty (50) pounds and exertion of up to twenty-five (25) pounds of force to move objects

ENVIRONMENTAL FACTORS
  • Occasional contact with persons accused or convicted of crimes
  • Occasional contact with or proximity to emergency medical situations
  • Exposure to dust, chemicals, fumes, and high noise levels

IRREGULAR HOURS
  • Work outside of the normal office hours (Monday-Friday 8 a.m. to 5 p.m.) may be required
  • May be deemed essential personnel in the event of an emergency
  • Regular attendance is required

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in Human Services, Criminal Justice, or a related field
  • Basic knowledge of record keeping
  • Working knowledge of Microsoft Office Suite, scanning and faxing operations

LICENSES AND CERTIFICATIONS
Required within thirty (30) days of employment
  • Valid driver’s license

PREFERRED QUALIFICATIONS
  • Qualified Mental Health Professional certification
  • Child and Adolescent Needs and Strengths (CANS)/Adult Needs and Strengths Assessment (ANSA)
  • Two (2) years of experience in providing crisis intervention, crisis assessment, and jail diversion services
  • Experience in records management and personal health information records
