We are seeking a Physical Therapist for a contract opportunity with Heart of the Rockies Medical in Salida, Colorado. This is a strong opportunity for a candidate with outpatient and orthopedic care experience who is looking for a steady rural healthcare clinic assignment in a beautiful Colorado location.

Responsibilities

• Perform comprehensive patient evaluations to assess impairments, functional limitations, and therapy needs
• Develop individualized treatment plans based on diagnosis, prognosis, and patient goals
• Provide outpatient and orthopedic physical therapy services within the PT scope of practice
• Design and modify therapeutic interventions to improve mobility, strength, function, and daily living activities
• Support injury prevention, wellness, fitness, and overall patient health education
• Maintain accurate and timely documentation, including treatment plans, progress notes, physician orders, attendance, and discharge summaries
• Collaborate with healthcare team members to support quality patient outcomes
• Ensure timely completion of therapy documentation and service-related charges
• Requirements
• Minimum 2 years of Physical Therapist experience required
• Outpatient and orthopedic care experience required
• Active Colorado Physical Therapist license required
• Current CPR certification required
• Graduate of an approved physical therapy program
• APTA membership or eligibility preferred
• Ability to work Tuesday Friday, 10-hour day shifts
• Strong clinical judgment, documentation skills, and patient-centered care approach
