Collaborative Care Manager (LPN/RN)

Job Overview
The Collaborative Care Manager will support our Collaborative Care Management (CoCM) program. The successful candidate will conduct monthly chart reviews to assess patient history, current care plan and identify any opportunities in care delivery to improve the management of various psychiatric diagnoses for patients residing in long-term care, assisted living, and rehabilitation settings. This role focuses on identifying care gaps and ensuring patients receive appropriate mental health support through structured care plans.

Role & Core Responsibilities:

  • Actively participant in the care team to review, augment and update care plans to be patient centered following patient goals of care and advanced care planning.
  • Identify and collaborate to identify care opportunities to improve health outcomes and enhance the quality of life for patients.
  • Utilize the ThoroughCare platform to document care plans and patient progress.
  • Ensure documentation is accurate, complete, and compliant with healthcare regulations.
  • Perform monthly chart reviews to assess patient progress and identify gaps in psychiatric care.
  • Monitor treatment adherence, including medication compliance and therapy participation.
  • Identify missed interventions, such as delays in psychiatric evaluations or medication adjustments.
  • Regularly review patient records for adherence to psychiatric treatment plans.
  • Ensure accurate and timely documentation in compliance with healthcare standards.
  • Communicate findings and recommendations to internal quality improvement teams.
  • Collaborate with Team Leads and RN Clinical Managers for clinical growth and development.
  • Meets established performance metrics and quality benchmark scores.
  • Support the Lumina Care mission to solve the problems in care delivery that slow down collaboration, coordination, and communication—causing care to be more complicated than necessary

Qualifications:

  • Active and unrestricted compact Licensed Practical Nurse (LPN) or Registered Nurse (RN) is required.
  • Experience in behavioral health, psychiatric nursing, long-term care, rehabilitation, or quality improvement is a plus.
  • Technology proficiency in electronic health records (EHR) and care management software (e.g., ThoroughCare, Sigma Care, and Matrix).
  • Strong attention to detail, analytical skills, and ability to identify care gaps.
  • Candidates are required to use their own equipment.

Pay: $27-$35/hr. Compensation is determined based on market range, experience, skills, education, and qualifications.
