The Larimer County Department of Health and Environment is seeking a Communicable Disease Nurse to perform disease and outbreak investigations as well as supporting our Senior Public Health Nurse and Medical Assistant in our Tuberculosis program.

In this role, you will conduct detailed case investigations for reportable conditions, with a primary focus on enteric and vaccine-preventable diseases. You will also play a key role in responding to and containing outbreaks in high-priority settings, including schools, childcares, and long-term care facilities.

This position combines investigative work with hands-on clinical duties to aid in disease mitigation efforts, including specimen collection and vaccine administration. A portion of the workload also involves specialized case management for Tuberculosis (TB), where you will coordinate care and monitor treatment plans for patients with active TB disease and Latent Tuberculosis Infection in accordance with public health protocols.

The standard work schedule for this position is Monday through Friday, 8:00 AM to 4:30 PM. This position may be asked to respond to events outside the typical work schedule. During the initial six-month probationary period, the individual hired may telework up to one day per week, if business needs allow. After successfully completing the probationary period, the individual may telework up to two days per week, depending on business plans and operational needs.

Responsibilities

What you'll be doing:

  • Perform Disease Investigations: Receive and review reports of notifiable conditions—specifically focusing on —by interviewing patients to determine symptom history, exposure sources, and transmission risks.
  • Lead Outbreak Response: Investigate and contain clusters of illness in high-risk settings such as schools, childcare centers, and long-term care facilities, providing guidance on isolation, exclusion, and environmental cleaning.
  • Manage Tuberculosis Caseloads: Coordinate comprehensive care for patients with active TB disease and Latent Tuberculosis Infection duties include monitoring medication adherence, assessing side effects, conducting home visits, and ensuring successful treatment completion.
  • Provide Clinical Services: Perform hands-on clinical tasks to support communicable disease investigations and outbreaks, including administering post-exposure prophylaxis, vaccines and collecting biological specimens for diagnostic testing.
  • Documentation & Surveillance: Maintain accurate, confidential records of investigations and clinical encounters, entering data into state surveillance systems to track community health trends.

What We're Looking For

  • Knowledge of Disease Processes: Understanding of transmission modes, incubation periods, and control measures for a variety of communicable diseases.
  • Investigative Interviewing Techniques: The ability to conduct sensitive, thorough patient interviews to elicit contacts, travel history, and exposure sources, often regarding private medical information.
  • Infection Control in Congregate Settings: Familiarity with regulatory guidelines and containment strategies specific to schools, childcare centers, long-term care facilities, and other congregate settings (e.g., exclusion criteria and isolation protocols).
  • Clinical Proficiency: Demonstrated skill in specimen collection and the application of immunization standards (schedules, storage, and handling).
  • TB Case Management Expertise: Ability to manage long-term, complex treatment plans, including interpreting chest x-rays/labs, monitoring for drug toxicity, and ensuring adherence over months of therapy.
  • Risk Communication: The ability to clearly explain complex medical concepts and isolation requirements to diverse audiences, including the general public, healthcare providers, and other community partners.

  • Bachelor's Degree, or equivalent combination of education and/or experience, in Nursing from an accredited college or university required. Related education or experience may be substituted on a year for year basis.
  • Licensed to practice as a registered nurse (RN) in the State of Colorado required.
  • One (1) year experience in providing health services as a Registered Nurse required.
  • Possession of CPR and First Aid certifications by date of hire required.
  • Possession of a valid Driver's License by date of hire required.
  • One year (1) of experience in specific nursing program relevant to the role they will perform preferred.

Supplemental Information

Please Note

The Department of Health and Environment has a vaccine requirement. Successful candidates will be required to comply with the vaccine requirements or request a waiver.

Employees will receive a 2% market increase on January 8, 2026. Employees who start before January 8, 2026, will be hired at the 2025 pay rate and will receive the 2% market adjustment on January 8, 2026, Employees who start on or after January 8, 2026 will be hired at the 2026 rate, which includes the market increase.

M ost positions are hired between min to mid range
