The Community and Clinic Social Worker serves as a bridge between clinical care and community support, ensuring patients have access to the resources and guidance they need both inside and outside the healthcare setting. This position provides psychosocial care, resource navigation, and care coordination for patients receiving treatment through oncology clinic partners, while also connecting individuals to community-based services that address the emotional, financial, and practical challenges of cancer.
Working closely with physicians, nurses, and care teams, the Social Worker helps identify and reduce barriers to care so patients can focus on treatment, healing, and quality of life. In partnership with Texas Oncology, this position provides on-site support at Fort Worth Cancer Center during afternoon clinic hours. The Social Worker works closely with clinic staff to respond to patient referrals, address psychosocial distress, and connect patients to supportive services that improve treatment adherence and overall well-being.
Essential Job Functions
· Conduct comprehensive assessments of clients and caregivers to identify social, emotional, financial and environmental needs.
· Provide ongoing emotional support to clients and caregivers as they navigate diagnosis, treatment, and survivorship.
· Develop individualized care and service plans, in collaboration with medical teams and other professionals.
· Provide ongoing case management by monitoring client progress, addressing barriers to care, and adjusting plans as needs evolve.
· Respond to crisis situations and provide short-term counseling and intervention as appropriate.
· Identify. coordinate, and connect clients to social services, benefits, financial assistance, and community-based resources.
· Advocate on behalf of clients and caregivers within healthcare and community systems to ensure access to needed services and equitable care.
· Educate clients and caregivers on patient rights, advance directives, and medical decision-making as appropriate.
· Communicate effectively with clients, caregivers, medical staff, and external partners to coordinate care and support smooth transitions.
· Maintain accurate and comprehensive records of patient assessments, interventions, and outcomes.
· Provide timely and thorough documentation for medical records, reports, and insurance claims.
Education, Licensure, and Professional Development
· Master’s degree in social work required with state licensure (LMSW).
· Strong interpersonal, written, and verbal communication skills.
· Demonstrated empathy, compassion, and commitment to serving individuals and caregivers impacted by cancer.
· Ability to work effectively with diverse populations and demonstrate cultural humility.
· Knowledge of social service systems, community organizations, and public assistance programs.
· Strong organizational, problem-solving, and crisis intervention skills.
· Proficiency with technology for documentation, data tracking, and communication.
· Experience in oncology, healthcare or medical social work preferred.
· After two years of employment, Cancer Care Services supports and covers costs associated with obtaining Oncology Social Worker Certification (OSW-C) in accordance with eligibility requirements.
Work Environment and Schedule
· Full-time position, Monday-Friday with occasional after-hours programming.
· Hybrid role serving both community and oncology clinic settings.
· Every other Friday Flex Schedule (half day off).
· Office-based, multi-staff environment with desk and necessary equipment provided.
· Reliable, independent transportation required for local travel.
· Benefits include health insurance, paid time off and 401(k).
· Office is closed one week in December for additional paid holiday time off.
Salary Range: $52,000 - $57,500
