Duties

  • Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology
  • Support clients with disabilities by offering personalized assistance and connecting them with appropriate services
  • Utilize motivational interviewing techniques to encourage positive behavior change and improve health outcomes
  • Collect and document data related to community health initiatives, ensuring accuracy and confidentiality
  • Deliver engaging public speaking sessions to educate groups on public health topics and available resources
  • Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans and follow-up services

Minimum Qualifications:

  • High school diploma or GED required.
  • Valid driver’s license and reliable transportation, required.
  • Ability to communicate effectively.
  • Strong organizational skills for health tracking, managing data, scheduling education, and working with community.
  • Ability to work independently in community-based settings.
  • CPR/First Aid certification

Preferred Qualifications:

  • College Education in Social Work, Nursing or Public Health
  • Community Health Worker certification.
  • Experience with rural populations or community-based outreach.

Pay: $22.17 - $26.69 per hour
