Duties
- Conduct outreach to community members to assess their health needs and provide tailored education on topics such as nutrition, addiction counseling, and preventive care
- Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology
- Support clients with disabilities by offering personalized assistance and connecting them with appropriate services
- Utilize motivational interviewing techniques to encourage positive behavior change and improve health outcomes
- Collect and document data related to community health initiatives, ensuring accuracy and confidentiality
- Deliver engaging public speaking sessions to educate groups on public health topics and available resources
- Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans and follow-up services
Minimum Qualifications:
- High school diploma or GED required.
- Valid driver’s license and reliable transportation, required.
- Ability to communicate effectively.
- Strong organizational skills for health tracking, managing data, scheduling education, and working with community.
- Ability to work independently in community-based settings.
- CPR/First Aid certification
Preferred Qualifications:
- College Education in Social Work, Nursing or Public Health
- Community Health Worker certification.
- Experience with rural populations or community-based outreach.

Pay: $22.17 - $26.69 per hour
