Community Liaison

To communicate the goals and service offerings of the Agency to the community. To facilitate and coordinate the transfer of patients from hospital or SNF and various health care organizations to the care of the Agency.
To help educate and inform the community, families, and clients about the services the Agency provides. To help clients and families with resources, support, and coordinate appropriate care.
To help build relationships and the Company.


Essential Functions:

  • Educates and communicates goals and service offerings to the community.
    • Makes in-person and telephone contacts with physicians, hospitals, health care agencies and similar organizations to apprise them of the availability of the company's services and to coordinate home care.
    • Makes presentations to a wide range of organizations to educate the public about Agency's home health care services.
    • Makes visits to current clients to evaluate and discuss client satisfaction with services.
    • Represents the company at appropriate meetings and organizations and acts as a liaison with other service providers in the public and private sectors.
    • Maintains accurate and complete records of all contacts made.
  • Acts as a liaison between the Agency and Facility Discharge Planning/ Social Services Department.
    • Reviews medical records according to hospital policy to assess home health needs of patients who are being discharged into home care.
    • Confirms that the Agency can meet the patient's needs.
    • Assists physicians, nurses, social workers, and other disciplines in establishing a home care plan prior to discharge.
    • Interviews patient/ family regarding home environment, current needs, and relevant psycho-social factors.
    • Makes referrals to Agency, including intake and background information, hospital course and care plan.
    • Maintains liaison relationship with hospital personnel, providing information and education on home health services.
  • Calls on physician offices on behalf of Agency.
    • Presents Agency services and offerings to physicians and their staff.
    • Picks up physicians orders and delivers orders for signature during the referral process whenever possible.
  • Actively participates in professional growth and development.
    • Participates in educational programs appropriate to functions.
    • Demonstrates current knowledge of home care needs of community and Agency's services.
    • Participate in the Quality Assessment and Improvement program.
  • Fulfills additional performance responsibilities as assigned and as necessary.


Physical Requirements: Visual/hearing ability sufficient to comprehend written/oral communication. Ability to deal effectively with stress.

Working Conditions: Works in office areas as well as in client homes and in assisted and independent living facilities. Sits, stands, bends, lifts, and moves intermittently during working hours. Interacts with clients, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances. Is subject to hostile and emotionally upset clients, family members, staff, visitors, etc. under all conditions/circumstances. Is involved in community/civic health matters/projects. May be exposed to infectious waste, diseases, conditions, etc., including exposure to HIV and hepatitis viruses. Communicates with the medical staff, nursing staff, and other supervisors. Maintains a liaison with clients, their families, support staff, etc. to assure that the clients' needs are continually met.

Qualifications: Medical knowledge with at least three (3) years in community or home health care environment. Communications, sales and marketing experience required.
