Duties & Responsibilities


SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Assist bariatric patients in navigating the pre-operative and post-operative care process, including required visits, testing, and documentation.

  • Provide patient education regarding program requirements, lifestyle changes, and follow-up care.

  • Conduct outreach and follow-up to reduce missed appointments and ensure completion of bariatric program requirements.

  • Coordinate with multidisciplinary team members, including surgeons, nutritionist, behavioral health providers, and primary care providers.

  • Assist with tracking patient progress through the bariatric surgery pathway, including nutrition visits, psychological clearance, laboratory testing, and medical clearances.

  • Connect patients with community resources and support services related to nutrition, behavioral health, and chronic disease management.

  • Support program operations by maintaining accurate documentation and patient tracking logs.

  • Participate in program initiative aimed at improving patient engagement and surgical outcomes.

Minimum Qualifications


1. Three (3) years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
2. Education and/or experience equivalent to 1. above. Completion of an approved six (6) month training program of combined classroom training and on-the-job experience in community liaison work may be substituted for six (6) months of experience. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of thirty (30) semester credits for each year of the experience described above. However, all persons must have at least one (1) year of the full-time experience described above.

Note: Preference will be given to applicants with at least a high school diploma or its educational equivalent approved by a State’s Department of Education or a recognized accrediting organization.

Department Preferences


  • High school diploma or equivalent required

  • Associate's degree or relevant coursework in public health, social services, or related field preferred.

  • Strong interpersonal and communication skills. Ability to work effectively with diverse patient populations.

  • Organizational skills and ability to manage multiple task.

  • Basic documentation and data entry skills.

  • Minimum of 1-2 years of experience in community outreach, patient navigation, or health
