Responsibilities:
Community Health Worker- OTP Clinic
The Community Health Worker (CHW) must be a frontline public health worker who is a trusted member of the community served. The CHW supports the Guilford County Sheriff’s Office Re-entry Program and Jail Diversion Program, which serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The CHW focuses on supporting formerly incarcerated individuals and those at risk of incarceration to successfully reintegrate into the community, reduce recidivism and improve access to healthcare, social services and supportive networks.
The Guilford County Sheriff's Office, in collaboration with Foundations Health High Point, will provide Medication Assisted Treatment (MAT) services for individuals detained in the Guilford County Detention Center who have a Substance Use Disorder (SUD).
Responsibilities:

Conduct outreach to individuals in jail, prison, or community settings to engage them in reentry and diversion services.

Facilitate connections to healthcare providers, mental health and substance use treatment, housing, employment, education, and social support services.

Provide culturally competent, trauma-informed peer support and advocacy to promote successful re-entry and diversion outcomes.

Assist clients in navigating healthcare systems, social service agencies, and legal resources.

Collaborate with case managers, social workers, probation officers, and community organizations to develop and implement individualized reentry plans.

Educate clients on health and wellness topics, including substance use recovery, mental health, and chronic disease management.

Maintain accurate documentation of client interactions, progress, and outcomes per program and regulatory requirements.

Participate in interdisciplinary team meetings, training, and community outreach events. Other duties as assigned

This position is supported by grant funding for an initial 18 month period, with the opportunity to extend for another 12 months. Following the grant period, there may be opportunities for the role to continue as part of ongoing operations, contingent on program sustainability and organizational needs.

Qualifications:

The CMH must have at least a high school diploma or equivalent with at least 4 years of relevant experience. Or a Bachelor’s degree in a related field.
- A valid drivers’ license and proof of auto insurance is required for this position. Reliable means of transportation to complete job functions.
