We are looking for a dynamic and compassionate individual to join our team as a Community Outreach Coordinator. This role plays a crucial part in expanding our reach within the community and ensuring that our services are accessible to those who need them most.

Job Summary:

As the Community Outreach Coordinator, you will manage referral sources and the referral database within our service territory. You will analyze quarterly data to identify opportunities, develop targeted strategies, and ensure accurate and up-to-date marketing materials. The role involves maintaining relationships with various referral sources such as assisted living and nursing homes, physicians’ offices, community organizations, and more. You will also represent Bridging Life Hospice at local events, develop community partnerships, and collaborate with key health system partners to support the delivery of coordinated care for the seriously ill and frail elderly.

Key Responsibilities:

  • Manage and maintain the referral database for the service territory.

  • Identify trends in data and create actionable strategies to drive growth and engagement.

  • Develop and implement a comprehensive marketing plan to promote hospice services.

  • Foster relationships with key referral sources, including assisted living and nursing homes.

  • Actively educate and promote hospice services to healthcare providers and community partners.

  • Conduct regular visits to physicians' offices and other provider locations.

  • Represent Bridging Life Hospice at community fairs, events, and presentations to civic organizations.

  • Coordinate and organize marketing events, presentations, and outreach efforts.

  • Collaborate with community partners such as Departments of Aging, palliative care providers, and faith-based communities to support coordinated care.

  • Work closely with LifeBridge Health and Carroll Hospital’s Physician and Provider Relations departments.

  • Ensure communication of relevant updates and information to internal teams for action.

Requirements:

  • Education: Bachelor’s degree in Nursing, Marketing, or a related field preferred.

  • Experience: Previous healthcare experience required.

  • Licensure: Registered Nurse (RN) License in Maryland or eligibility to obtain Maryland licensure is preferred.

  • Skills:

    • Strong communication and relationship-building abilities.

    • Ability to develop and implement effective marketing strategies.

    • Familiarity with healthcare systems and referral processes.

    • Self-starter with strong organizational skills and attention to detail.

    • Ability to work independently and as part of a collaborative team.
