Job Summary:
The Community Partnerships Coordinator (CPC) is responsible for building and managing relationships and partnerships to address the social determinants of health and to improve community health and health equity. These relationships and partnerships include employees, community members and key stakeholders in our service area.
The CPC is responsible for networking and representing Grand Itasca in community settings to keep abreast of emerging community issues, strengthen community relationships, and link individuals and organizations with Grand Itasca to help create optimal health and well-being for our community.
The CPC provides direction and support to staff and programming, with a lens focused on improving health equity through addressing the social determinants of health.
Here's what you'll do when you join us:

Manages community relations contacts, processes, and committees both internally and externally. Partners with Fairview Community Advancement teams to oversee processes including Community Health Needs Assessment (CHNA), CHNA Implementation Strategy Report, and Hospital Action Plans.
- Represents Grand Itasca in a variety of community venues and serves on community committees/boards to develop strategic relationships, maintain an understanding of local community ecosystem and politics, and keep abreast of emerging community issues. Champions Grand Itasca programs and resources. Navigates and makes key connections for community partners to Grand Itasca staff and services. Develops relationships with organizations, institutions, and members of the community through networking efforts, partnerships, and collaborative activities. Serves as a bridge between community partners and healthcare, by way of a consistent, trusted voice.

Supports strategies to ensure community health and health equity by addressing age, gender, race, culture, sexual orientation, ability, geography, religion, and other factors influencing health.

Liaise with Communications and Marketing staff to support internal employee and external community relations, PR, and communications.
- Contributes to data tracking and collection processes for community benefit reporting, utilizing a standardized tool for such purposes. Maintains proficiency in data collection and understands the importance of reporting as it affects hospital viability.
- Responsible for performing work at multiple Fairview or community sites.

Required

Bachelor’s degree in a related field such as public health, nursing, community health etc. or a combination of equivalent education and experience.

Demonstrated previous community relation and community engagement experience.

Ability to develop a network of relationships with diverse populations and work across race, class, sector, and geography.

Has a keen understanding of the sociopolitical environment in Minnesota.

Ability to follow and adhere to Grand Itasca’s policies, mission, and values.

Maintain reliable and consistent attendance.
-
Must be able to work independently and be self-guided.
Preferred
-
Three years’ experience in a community relations related field.

Extensive experience and relationships in the Grand Itasca service area.
