Medical Records Documentation & Data Entry

Job Purpose:
To accurately write, update, and maintain patients’ medical records in a systematic and timely manner, ensuring completeness and accuracy of documentation for quality patient care and compliance.

Key Responsibilities:

· Record patient information, diagnoses, treatment details, and medical history into files and hospital information systems.

· Regularly update medical records with progress notes, reports, and discharge summaries.

· Verify accuracy of entries and ensure all required forms and documents are properly completed.

· Maintain orderly filing and digital updating of patient data for easy retrieval.

· Assist doctors, nurses, and hospital staff in documenting clinical details as required.

· Ensure records are updated promptly after patient visits, admissions, or procedures.

· Correct errors or missing information in consultation with relevant medical staff.

Job Types: Full-time, Fresher

Pay: ₹15,000.00 - ₹18,000.00 per month
