Contract Case Manager – Behavioral Health / Substance Use Services

Empower Recovery is seeking a qualified Contract Case Manager to support individuals receiving substance use and mental health treatment in our outpatient and IOP programs. This role focuses on care coordination, resource linkage, and collaboration with clinical and peer support teams to promote client stability and recovery.This is an ideal opportunity for experienced professionals who prefer flexible, contract-based work in behavioral health.

Key Responsibilities:

Provide case management and care coordination for clients in outpatient and IOP services

Collaborate with clinicians, peer coaches, probation officers, courts, and community partners

Assist clients with housing, employment, benefits, and community resources

Develop and monitor individualized service plans

Maintain timely and accurate documentation in compliance with Colorado BHA and Medicaid standards

Participate in multidisciplinary team meetings

Support client engagement and continuity of care

Qualifications Required:

Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent experience)

Certified Addiction Technician (CAT) or ability to obtain certification

Experience in behavioral health, substance use treatment, or social services

Strong organizational, communication, and advocacy skills

Ability to work independently as a contractor

Preferred:

Experience with Medicaid and BHA-compliant documentation

Knowledge of ASAM criteria and recovery-oriented systems of care

Experience working with justice-involved populations

Familiarity with telehealth platforms and electronic health records

Schedule & Work Structure:

Flexible schedule based on program needs

Contract role with variable hours

Compensation:

1099 independent contractor position

Competitive contract rate based on experience and workload

Pay: $23.00 - $24.00 per hour
