Role Overview

We are seeking an experienced pelvic floor physical therapist to provide in-home treatment for a female patient in her mid-80s presenting with urinary incontinence. The patient is mobile and requires consistent, skilled care 2–3 times per week.

Key Responsibilities

• Deliver comprehensive pelvic floor physical therapy focused on female urinary incontinence
• Perform thorough evaluations and develop individualized treatment plans
• Implement hands-on treatment, neuromuscular re-education, and functional retraining
• Provide patient education and caregiver guidance as needed
• Maintain clear, concise documentation after each session
• Communicate progress, challenges, and updates in a weekly check-in with the principal clinician

Qualifications

• Licensed Physical Therapist (PT/DPT) in New York State, in good standing
• Extensive experience in pelvic floor therapy for female incontinence
• Experience treating geriatric populations strongly preferred
• Strong clinical reasoning and ability to adapt care to an older adult population
• Professional, punctual, and reliable with excellent bedside manner

• Must carry active liability and malpractice insurance
• Must have reliable transportation and ability to travel to Kings Point, Nassau County
• Must be able to commit to 2–3 consistent visits per week
• Comfortable working in a private home setting within a concierge model
• High level of accountability and communication
