The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:
The Community Outreach Coordinator will lead and report on activities to reduce health disparities for patients/residents within the Rahway and Trinitas service markets.
The coordinator will use the Community Health Assessment plan findings as well as each Hospital Leadership strategic plan as a quality and safety guide to assess the community's health and social needs.

Qualifications:
Preferred:
  • Bachelor's Degree in Business, Healthcare or other related field/experience preferred
  • Travel is required.
  • Participation is required during evening and weekend events.
  • Must have knowledge of and be able to operate audiovisual equipment.
  • Experience in delivering community education programs preferred.

Scheduling Requirements:

  • Full time days
  • 8-4:30
  • Nights and weekend requirements for events.
Essential Functions:
  • Coordinator is responsible for the creation of programs, tracking of information, through various outlets, about healthcare resources and support services
  • Design, coordinate and staff RWJBH health programs, including but not limited to, RWJBH fitness and wellness center events such as: provider events, dietary and cooking classes, support groups, etc.
  • Design, coordinate and staff community programs, including but not limited to, libraries, senior centers, AARP, churches, health fairs, social clubs, schools, YMCA, annual meetings, local pantry and other hospital events.
  • Collect RSVPs, send email reminders, collect attendance sheets and file as needed.
  • Work collaboratively with on-site and system assigned marketing staff for materials, content, flyers, newsletters, promotional materials, advertisements.
  • Meeting all set deadlines.
  • Participate in the community health needs assessment coordinated by the system, Completed every three years.
  • Participate in the creation of, and execute, the community assessment plan,
  • Work with hospital leadership for developments, revisions and updates annually.
  • Participate in the systemwide Social Impact & Community Investment Program and Committee
  • Timely entry of all events for both sites into the online CIBSA portal.
  • Provide status updates to leadership as needed on the Community Health activities and CHNA progress.
