GluCare Integrated Diabetes Center is seeking a dedicated DHA Nurse to join our clinical team in delivering exceptional patient care. Key Responsibilities:
Patient Care Support:

Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations

Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans
Clinical Procedures Assistance:

Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards

Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection
Patient Education and Communication:

Communicate effectively with patients, providing empathy and clarity to address their questions and concerns

Reinforce health education provided by physicians, encouraging self-care practices and adherence to treatment plans
Documentation and Reporting:

Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR)

Report any changes in patient condition to the supervising nurse or physician promptly
Operational and Safety Compliance:

Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well-being of patients and staff

Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support
Requirements
Qualifications:

Valid DHA Nurse License - A MUST

Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management

Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred

Strong communication and interpersonal skills with a patient-centric approach

Basic proficiency in electronic medical records (EMR) systems is preferred
