Job Description: As a Direct Care Worker, you will be responsible for providing essential care and support to clients, helping them with daily living activities, and ensuring their comfort and well-being. You will play a vital role in improving the lives of our clients by offering personalized care tailored to their specific needs.

Key Responsibilities:

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding.
• Provide mobility assistance, including helping clients move around safely.
• Monitor and record the client's health status, including vital signs, and report any changes to the healthcare team.
• Administer medication as prescribed and assist with medication reminders.
• Prepare and serve meals according to the client’s dietary needs and preferences.
• Perform light housekeeping tasks such as laundry, cleaning, and organizing.
• Accompany clients to appointments, errands, and recreational activities as needed.
• Build and maintain positive relationships with clients, families, and other caregivers.
• Follow the care plan and communicate effectively with the healthcare team to ensure comprehensive care.
• Maintain accurate and up-to-date documentation of care provided.

Qualifications:

• High school diploma or equivalent.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is preferred but not required.
• Previous experience in home care, elderly care, or related field is highly desirable.
• Ability to lift and move clients as needed.
• Strong communication and interpersonal skills.
• Compassionate, patient, and empathetic demeanor.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver’s license.
• CPR/First Aid certification is a plus.
• Ability to pass a background check and drug screening.
• Must be able to read and write.
