Salary: $65K ; Depending on experience

Qualifications:

• Must have Case Management experience with assisted living
• Must have clinical knowledge
• Must have prior experience in geriatric population
• Must have a valid New York RN license
• Must have experience in Assisted Living
• Strong written and verbal communication skills

Responsibilities:

• Oversee and manage all aspects of case management for the facility, ensuring compliance with state and federal regulations.
• Develop, implement, and monitor individualized care plans for residents in collaboration with medical staff, families, and caregivers.
• Act as a liaison between residents, families, healthcare providers, and community resources to facilitate optimal care and services.
• Train, lead, and support a team of case managers and related staff to maintain high standards of care.
• Monitor residents’ progress and make necessary adjustments to care plans to ensure their needs are consistently met.
• Maintain accurate and up-to-date documentation and reports related to case management activities.
• Address and resolve any concerns or issues related to resident care in a professional manner
