Job Summary/Objective:

Provide leadership and direction to the Health Center nursing team members to ensure that high quality nursing care is provided for all Health Center residents, which includes the Chateau (Long-Term Care and Skilled Nursing Program), the Villa (Assisted Living Program), and the Abbey (Memory Support/Assisted Living Program), through oversight of daily operations, operational planning, and regulatory compliance.


Supervisory Responsibilities:

  • Supervises all clinical team members.
  • Recruits, interviews, hires, and trains new staff.
  • Oversees the daily workflow of the department.
  • Provides constructive and timely performance evaluations.
  • Handles discipline and termination of employees in accordance with company policy.

Essential Functions/Duties/Responsibilities:

  • Lead and manage direct reports to assure the desired level of resident service, care, and satisfaction. Monitor and evaluate team member performance; provide coaching, training, and corrective actions as required. Assure equitable administration of policies, and compliance with regulatory and legal requirements. Develop and provide appropriate recommendations for the full range of team member and organizational matters, including staffing, compensation, promotion, and termination actions.
  • Develop, implement, and sustain department specific tools, measures, and methodologies to maximize individual and team efficiencies, productivity, and effectiveness. Specific initiatives include organizational and job design, cross-training, succession planning, overtime control, and emergency/critical event preparedness.
  • Oversees weekly and monthly clinical task completion to include pharmacy and consultant reports and recommendation follow up, discharge medication destruction, physician order reviews, and review service plans.
  • Review Quality Measures to ensure accurate assessments and resident coding for best outcomes.
  • Contribute to the Leadership Team to facilitate strategic, financial, and operational plans. Promote and develop positive and professional interaction with peers through effective written and verbal communication, listening, and providing feedback. Collaborate with Leadership Team members to facilitate and assure a seamless relationship with nursing team members.

  • Partner with the other Leadership Team members and Human Resources to create and implement a variety of training, learning, and coaching programs, and opportunities, designed to meet the specific business needs of the Health Center. Specific initiatives involve areas such as leadership and team member development, team member relations and communications, performance management and enhancement, teamwork, and safety.
  • Review state and federal regulations to ensure that Montereau remains compliant with regulatory requirements. Initiate corrective actions and/or operational plans or procedures and communicate these as appropriate to remain compliant with changing regulations and industry practices.
  • Facilitate a positive communication process and rapport between residents, their families, and nursing team members to ensure the needs and concerns of all parties are effectively addressed to provide high quality nursing care as assessed by satisfaction surveys and comments.
  • Provide oversight and guidance to assure a dynamic Quality Assurance (QA) program for the Health Center that effectively identifies problem areas (e., infection control, weight loss, medication administration, etc.) and provides tracking and trending of issues through effective resolution.
  • Manage incident resolution for the Health Center and assure that reporting incidents to the Oklahoma State Department of Health (OSDH) and the Department of Human Services (DHS) are completed timely and correctly in accordance with regulations.
  • Oversee and participate in the daily staffing to include new team member interviews, hiring, orientation and scheduling to ensure positions are filled and adequate to deliver exceptional resident care. Monitor’s area HPPD and ensure they are in line with budget.
  • Participates in the MDS process by signing the assessments as RN.
  • Attends interdisciplinary team meetings to include weekly Standards of Care, Medicare, Leadership, and resident care conferences.
  • Attend and participate in regular meetings with all Health Center departments, residents, and families to remain up to date on all operational areas to provide a rapid response to all known and anticipated issues and problems that might affect nursing care.
  • Assist in developing the annual budget for all elements of the Health Center nursing operations.
  • Participate in the Nurse Manager On-Call rotation to support Nurse Care Partners with critical resident care decisions and assure adequate staffing and scheduling.
  • Work care base shifts, as needed, to ensure proper resident care is provided.
  • Act as RN consultant for Assisted Living departments.
  • Participate in the Leader on Duty (LOD) rotation.
  • Perform other related functions as required or as assigned.

  • Strong knowledge related to establishing and following care plans.
  • Strong resident care documentation skills.
  • MDS knowledge preferred.
  • Working knowledge of Microsoft Office programs and ability to learn new software systems (i.e., Point Click Care).
  • Effective group and individual training, performance coaching, and teambuilding skills.
  • Ability to work a variety of shifts (days, evenings, nights, weekends), when needed.
  • Strong communication skills to effectively and empathetically address critical health and death issues with a diverse resident population and family members.
  • Strong interpersonal skills and understanding of quality customer management and service
  • Ability to effectively interact with a diverse leadership team, staff, residents, and third-party associates, including vendors, providers, and visitors.

Education and Experience

  • A minimum of 5 years of skilled nursing experience within a long-term care (LTC), assisted living, or hospital environment is preferred.
  • Previous experience working with federal, state, and local nursing services regulations and practical experience analyzing and interpreting statistical reports while working in an interdisciplinary team environment is required.
  • Previous leadership and administrative experience in a healthcare setting (i.e., schedules, budget, reporting, etc.) is required.
  • Previous experience within a Life Plan Community is preferred.
  • Current State of Oklahoma Registered Nurse license is required.
  • Bachelor’s degree in nursing is preferred.

Physical Requirements:

  • An extensive amount of walking, standing, and sitting, with a moderate amount of bending at the waist, kneeling, twisting/turning, reaching above the head and shoulders, and eye-hand coordination
  • Ability to routinely lift 35 pounds and push wheelchairs with loads of up to 300 pounds
  • Frequent use of ordinary communication, including normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision, and depth perception
  • Ability to use a computer or other office equipment for an extensive period of time
  • Ability to occasionally travel by car and/or commercial airline
