Director of Nursing – Home Infusion Services

Location: Texas (DFW preferred; Houston or Austin considered)
Travel Requirement: Regular travel to Houston during initial launch phase; ongoing travel within Texas as needed
Reports To: Senior Director of Specialty & Infusion

Position Summary

The Director of Nursing (DON) will serve as the clinical and regulatory leader for a startup home infusion program operating in conjunction with a specialty pharmacy in Texas.
Operations will initially launch from an existing Houston-based LTC location. The DON will provide onsite oversight during startup, establish nursing infrastructure, and support accreditation readiness during the launch phase.
The organization anticipates securing a permanent site in the Dallas–Fort Worth area. As the platform expands — including potential development of Ambulatory Infusion Suite (AIS) or Alternate Infusion Center (AIC) services — this role will provide clinical oversight across sites within Texas. Long-term residence within Texas is flexible, with preference for candidates based in DFW.
This role may also serve as the designated HCSSA Administrator, if required, in accordance with Texas regulations.
This is a startup leadership role requiring hands-on program development, regulatory oversight, and scalable clinical infrastructure buildout.

Core Responsibilities

Regulatory & Compliance Leadership

  • Serve as the designated Director of Nursing under Texas HCSSA regulations.
  • May serve as HCSSA Administrator if designated.
  • Ensure compliance with all applicable state and federal home health regulations.
  • Develop and maintain nursing policies and procedures.
  • Lead ACHC IRn accreditation preparation and survey readiness.
  • Establish and oversee Quality Assurance and Performance Improvement (QAPI) program.
  • Ensure documentation compliance and audit readiness across all operating sites.

Clinical Program Development

  • Develop infusion therapy protocols (IVIG, biologics, specialty therapies).
  • Establish patient assessment, care planning, and documentation standards.
  • Define clinical escalation and emergency response procedures.
  • Oversee infection control processes and patient safety standards.
  • Collaborate with pharmacy leadership to align clinical and dispensing workflows.
  • Support clinical development of AIS/AIC services at the permanent site, if implemented.
  • Collaborate with pharmacy leadership if sterile compounding services are introduced.

Staffing & Clinical Leadership

  • Recruit, hire, and train infusion nurses.
  • Validate and document nursing competencies.
  • Establish 24/7 on-call clinical coverage model.
  • Oversee subcontracted nursing relationships during launch phase.
  • Provide direct supervision, coaching, and performance management.

Operational Collaboration

  • Partner with PIC and pharmacy leadership to support time-to-therapy goals.
  • Participate in referral education efforts as needed.
  • Support multi-site growth and operational transitions.

Qualifications

Required

  • Active Texas RN license in good standing.
  • Eligible to serve as Director of Nursing under Texas HCSSA regulations.
  • Eligible to serve as HCSSA Administrator if designated.
  • Minimum 5 years clinical experience.
  • Minimum 2 years leadership or supervisory experience.
  • Experience in infusion therapy (IVIG, biologics, specialty therapies).
  • Experience with regulatory surveys (ACHC, CMS, or Joint Commission).
  • Willingness to travel regularly within Texas during launch and expansion phases.

Preferred

  • Experience in home infusion startup or program build.
  • Experience supporting infusion suite or ambulatory infusion services.
  • Experience collaborating with specialty pharmacy operations
