Disability Case Manager

Arc of Maui County is a non-profit agency established in 1954 that promotes and protects the human rights of people with intellectual and developmental disabilities. We actively support programs and services that assure full inclusion and participation in the community throughout life with Residential, Home & Community Based, Adult Day Health and Employment Services.

We are seeking a Case Manager to oversee services for a small caseload of individuals in the Home & Community Based program and directly support individuals to meet their program goals. Applicants must possess strong interpersonal, supervisory, organizational skills, and be proficient in Microsoft Office programs.

Shift - Mon-Fri 8a-4p

Responsibilities Include: Development of individual program plans, supervision and training for program implementation, participation in annual inter-disciplinary meetings, monthly reviews, and writing monthly/quarterly reports.

Applicant requirements: Bachelor's Degree in Human Services or related field or Bachelor's Degree in another field (with one year experience working with individuals with developmental disabilities or elderly), or Registered Nurse. Must be able to pass a criminal background check and drug test. TB, CPR/FA required upon hire (will assist to obtain).
