Driver (Part-time) Mornings or Late Afternoons for Non-Emergency Medical Transportation

At this time, we are looking for part-time. High need during mornings beginning 5:30 - 6 AM am and later after 2 pm.

The Charak Center for Health and Wellness is a well-established, state certified and Joint Commission accredited behavioral health agency with locations in Elyria, Garfield Heights, Medina, Mentor, and Stow. We provide services to individuals with mental health and substance use issues.

This job is for driving eligible Lorain County residents to and from appointments. Additionally, when not transporting, will provide behavioral health case management services to agency clients.

Our program depot is our Elyria office.

Our agency provides behavioral health and substance abuse recovery services including but not limited to psychiatric services including medication assisted treatment for behavioral health and substance abuse treatment needs; behavioral health nursing services; individual therapy; group treatment; intensive outpatient treatment; mental health day treatment; and case management services.

Credentials (Education/Experience/Licenses):

OSHA Category: This position may perform tasks that involve exposure to blood, body fluids, or tissues. All employees are offered to opportunity to receive the Hepatitis B vaccination series at the Practice’s expense. Must be able to lift 50 pounds in an emergency.

Must complete driving certification course within 30 days of hire.

ESSENTIAL FUNCTIONS/JOB DESCRIPTION:

- Transport eligible Lorain County residents to and from individual/group appointments as scheduled by the Lorain County Department of Job and Family Services..

- Attentive to traffic and weather conditions while staying on schedule and ensuring the safety of patients.

- Ensure that their vehicles operate properly each day by conducting safety checks including testing brakes, tires and other components.

- Responsible for maintaining a safe environment within vehicles by enforcing rules of conduct and responding to emergencies.

- Keep track of the trips, mileage and amount of fuel used. Verify riders, obtain rider signatures. Complete and maintain required documentation.

- Complete appropriate notification of crises and emergencies. Complete appropriate related documentation.

- Performing a case management functions such as linking, referring, coordinating, and managing the delivery of services to the client in relation to the established goals and objectives in their treatment plan

- Document client information using paper/electronic records

- Working knowledge of computers

- Conducts other duties as assigned.

Job Type: Part-time, PRN.. Our transportation program operates from 5:00 AM to 11:00 PM, seven (7) days per week. Position has potential to be ful-time based on performance.

Required education:

• High school or equivalent

Required experience:

• At least 21 years of age with a minimum of 2 years of good driving experience. with clean record, clean background check and completed drug screening.
• computer documentation: 1 year (Required)
• shuttle/bus driving: 1 year (Preferred)

Required license or certification:

• Possess a valid Ohio Motor Vehicle Operator's License and/or Commercial driver's license with passenger.
