Position Overview:

Residential Transitions, Inc. (RTI) is a growing social services organization dedicated to embracing, supporting, and honoring each person’s individuality within our programs. We serve the Minneapolis-Saint Paul area through various locations across the Twin Cities. As a Dual Case Manager (TCM & Waiver), you will be responsible for providing person-centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions. These individuals are eligible to receive services through the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver, as well as Targeted Case Management services. The Dual Case Manager will assess needs, identify goals, monitor progress, develop plans, and coordinate services for individuals on their caseload.

Role Responsibilities:

  • Manage and maintain a caseload of individuals diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions across Dakota County.
  • Assess needs, identify goals, and plan services to meet individuals’ needs and enhance their goals.
  • Visit clients at least once a month and twice annually in accordance with TCM and waiver-specific rules.
  • Coordinate client intake and discharges within set parameters and timelines.
  • Collaborate with other professionals such as physicians, mental health providers, and other teams to develop and update plans for each client.
  • Ensure financial authorizations and provider contracts are in place.
  • Research and identify service options to meet clients’ needs, and make referrals to medical, social, financial, housing, and other related services.
  • Communicate consistent information and ensure continuity of service for individuals.
  • Document services in individual records so that information is readily available and record-keeping standards are met.
  • Provide flexible, individualized support to help individuals lead self-directed lives.
  • Access community and housing resources to match the individual’s needs and preferences.
  • Facilitate communication among those involved to provide coordination and ensure service direction aligns with individuals’ goals.
  • Provide crisis assessment and intervention to help individuals stay safe in their homes and communities.
  • Promote an inclusive, diverse, culturally competent, and respectful workplace.

Required Qualifications and Experience:

  • The Case Manager shall be qualified under Minnesota law as follows: ·
  • Holds a Bachelor’s Degree in one of the behavioral sciences or related fields from an accredited college or university and has 2,000 hours (1 year) of experience in the delivery of service to persons with mental illness; OR
  • Master's Degree in behavioral science or related field
  • A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance.
  • Must be ambulatory, able to negotiate stairways, and able to lift and carry items up to or exceeding 40 pounds.
  • Certification in CPR and First Aid, or the ability to obtain certification upon hire.
  • Knowledge of person-centered planning, harm reduction, and the needs of individuals with serious mental illness/chemical dependency (SPMI/CD).
  • Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.

Preferred Qualifications and Experience:

  • Master’s degree in behavioral science or related field.
  • Experience providing direct services to adults with serious and persistent mental illness.
  • Experience with case management and commitment proceedings.

Compensation: $52,000 - $56,000 per year
Type: Full-Time, Exempt
Location: Dakota County – Hybrid
Physical & Mental Requirements:

Employees in this role must be able to:

  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques
  • Use hands for writing, typing, and data entry
  • Lift up to 40 lbs. and occasionally exert physical effort during duties
  • Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle
