Emergency Shelter Case Manager

Job Purpose/Summary

Case Manager will be responsible for screening referrals and enrolling individuals and families in the Emergency Shelter program. The program will provide short-term shelter placement for intact families, fathers with daughters, disabled, and/or transgender individuals who cannot be placed in traditional mass shelters. The Case Manager may provide ongoing case management and in-shelter services to clients in order to ensure success.

Duties and Responsibilities

List the primary job duties and responsibilities; provide specific examples of the activities required for the position.

  • Provide information, financial assistance, referrals, and professional case management for individuals and families entering Emergency Shelter.
  • Provide a case plan, case notes, and case goals for each client seen. If a client is no longer active with a scheduled-follow up meeting, then the file will have a closing note.
  • Document all client contact in real-time case notes that are clear, coherent, complete, correct, and conversational.
  • Input appropriate information into the HMIS database in real time.
  • Maintain superior case files with current (less than 30 days old at time of any service) information, including residency, income verification, and identification documents for all clients in the family unit.
  • Compile client demographic information for the previous month and turn it in to the supervisor by the 3rd business day of the new month.
  • Attend appropriate trainings that will enhance the professional abilities and methodologies used in the case management of clients.
  • Attend community outreach events such as health fairs, homeless service events, Henderson City events, and mainstream trainings to educate the public about HopeLink services.
  • Sign out of the office for any meetings or home visits with the destination and expected return time.
  • Maintain a call log to document the time and frequency of returned phone messages.

Qualifications and Competencies

Qualifications include:

  • Preferred Bachelor’s degree in Social Work, Education, Psychology, or related field
  • At least two years of case management experience
  • Basic computer skills
  • Time management and organization skills
  • Ability to establish and maintain cooperative working relationships with co-workers and clients
  • Reliable Vehicle, Nevada Driver’s License, and current automobile registration and insurance

Work Environment

  • The hours are Monday -Thursday from 7:30 am-5:30 pm with one- hour lunch.

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. This position is required to perform a variety of duties, often changing from one task to another, and to perform with frequent interruptions and/or distractions. In addition, this role requires travel via personal vehicle to and from clients’ homes for in-home services.

Physical Demands

  • Must be able to lift at least 30 pounds.
  • Must be able to sit for extended periods, stand, reach, and bend.
  • Regular travel within the community for shelter visits and meetings.

Pay: $57,000.00 per year
