Job Summary:

As an Enrollment Specialist, you play a critical role in guiding potential participants through the enrollment process. You evaluate eligibility, coordinate assessments, and manage required documentation to ensure timely, accurate enrollment into the program.

This position is ideal for someone who is analytical, organized, and comfortable working independently with detailed information. You will balance technical accuracy with thoughtful problem-solving, working closely with internal teams and external partners to move enrollment forward efficiently and correctly.

Schedule:

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:

  • Evaluate potential participants to determine eligibility and program fit.
  • Complete initial assessments, including home-based assessments as required.
  • Coordinate enrollment activities with state health and human services personnel.
  • Work closely with the interdisciplinary team to support initial assessments and enrollment decisions.
  • Follow established intake processes to ensure timely enrollment throughput.
  • Collect, complete, and maintain accurate enrollment documentation and required records.
  • Track and maintain enrollment data, metrics, and reports.
  • Participate in weekly meetings to review goals, data, and performance metrics.
  • Identify barriers to enrollment and support action planning to address challenges.
  • Participate in strategic planning related to enrollment goals and outcomes.
  • Support marketing and outreach activities as needed.
  • Participate in training, team meetings, and quality improvement initiatives.
  • Other duties as assigned.

Education and Experience:

  • Bachelor's Degree Required
  • One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
  • Knowledge of Medicare/Medicaid regulations.
  • Knowledge of sales and marketing strategies.

Physical Requirements:

  • Must be able to move intermittently throughout the workday.
  • Sits, stands, bends, lifts, and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is subject to lifting, carrying, and supporting Participants.
