Responsibilities

• Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to establish accurate diagnoses.
• Develop and implement personalized treatment plans for acute illnesses, chronic disease management, and preventive care initiatives.
• Utilize EMR (Electronic Medical Records) systems like Dr. Chrono for accurate documentation of patient encounters and medical records while ensuring compliance with HIPAA regulations.
• Engage in health coaching and patient education to promote lifestyle modifications, medication adherence, and disease prevention strategies.

Qualifications

• Valid TX. licensure as a Family Nurse Practitioner with current certification from the American Nurses Credentialing Center (ANCC) or equivalent.
• Proven experience in primary care settings with familiarity in managing pediatric through geriatric populations.
• Proficiency in EMR/EHR system.
• Experience working with diverse patient groups including those in assisted living, nursing homes, hospice & palliative care, memory care units, and behavioral health environments.
• Knowledge of CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-10 standards for billing and documentation purposes.

Pay: $105,000.00 - $175,000.00 per year
