ABOUT THIS OPPORTUNITY

Heartline Staffing is representing a Federally Qualified Health Center (FQHC) serving a 50-town rural region across western New York and Pennsylvania.

This is a full-spectrum outpatient Family Medicine position providing care to patients of all ages, with a strong emphasis on preventive medicine, chronic disease management, and continuity of care.

COMPENSATION & BENEFITS

• $280,000 – Five-day clinic schedule
• $250,000 – Four-day clinic schedule
• Compensation negotiable based on experience
• Sign-on bonus available

PRACTICE MODEL

• Outpatient Only
• 16–20 Patients Per Day
• 1:1 Dedicated Staffing Support
• Flexible 4- or 5-Day Clinic Schedule
• No Required Weekends (Optional Available)
• Phone Call Only – Approximately 5 Days Per Month (Third String)
• In-Office Procedures Based on Experience & Preference
• EMR: eClinicalWorks
• Teaching affiliation with UB Family Medicine Rural 1–2 Track Residency Program

Requirements:

REQUIREMENTS

• M.D. or D.O. from an Accredited Medical School
• Board Certified or Board Eligible in Family Medicine
• Licensed or Eligible for Licensure in Pennsylvania and/or New York
• Commitment to High-Quality, Patient-Centered Care
