Family Nurse Practitioner

ProSource Talent is excited to be hiring for a culturally competent Family Nurse Practitioner or Primary Care Nurse Practitioner!

We are seeking a Primary Care Nurse Practitioner (NP) to join a highly collaborative, community-based health center delivering comprehensive care to diverse and underserved populations.

This is an opportunity to practice purpose-driven medicine—supporting individuals across LGBTQIA+ communities, people living with HIV/AIDS, women’s health populations, immigrants, and individuals experiencing homelessness—while working within an integrated, multidisciplinary care model.

What You’ll Do

• Provide comprehensive primary care (preventative, acute, and chronic care) across in-person and telehealth settings
• Manage complex conditions including HIV, Hepatitis C, substance use disorders, and chronic illnesses
• Develop and oversee individualized care plans in collaboration with the care team
• Deliver patient-centered care with a strong focus on health equity and cultural competence
• Coordinate care across medical, behavioral health, and social service providers
• Provide patient education, counseling, and ongoing care management

What We’re Looking For

• Licensed Nurse Practitioner (NP) in New York
• Board Certified (AANP or ANCC) + BLS certification
• Experience in community health, FQHCs, or underserved populations strongly preferred
• Clinical experience in HIV care, primary care, or complex/chronic conditions
• Passion for working with diverse populations and advancing health equity
• Bilingual (Spanish or French) is a plus

Pay: $128,703.15 - $150,000.00 per year
