Job Summary

Duties

• Conduct thorough physical examinations and health assessments for diverse patient populations, including pediatrics, geriatrics, and individuals with complex medical conditions.
• Develop and implement individualized care plans, incorporating evidence-based practices and patient preferences.
• Manage acute and chronic illnesses through medication management, diagnostic evaluation, and health coaching.
• Utilize electronic health record (EHR) systems such as Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently.
• Perform procedures including suturing, IV infusion, catheterization, phlebotomy, and sterile processing techniques as needed.
• Coordinate with specialists for referrals or specialized procedures like gastroscopy or spinal tap when necessary.
• Provide triage services via telehealth or in-person visits to assess urgent needs and prioritize care delivery effectively.
• Educate patients on disease prevention, medication administration, nutrition, and lifestyle modifications to promote wellness.
• Assist with discharge planning, case management, and utilization review to ensure comprehensive patient support across settings such as hospitals, nursing homes, assisted living facilities, and home care.
• Maintain compliance with HIPAA regulations and ensure accurate medical documentation in accordance with NCQA standards and CPT coding guidelines.

Experience

• Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
• Demonstrated experience working in primary care settings with diverse patient populations including pediatrics, geriatrics, behavioral health, or acute care environments.
• Proficiency with EMR/EHR systems such as Epic, Athenahealth, Cerner, or eClinicalWorks is essential.
• Prior experience performing procedures like suturing, dermal fillers (nice-to-have), IV insertion/infusion therapy, catheterization, and basic life support certification required.
• Experience managing complex cases involving hospital medicine, ICU or trauma center environments (Level I/II Trauma Center experience preferred).
• Knowledge of ICD-10/ICD-9 coding systems along with CPT coding for billing purposes is highly desirable.
• Familiarity with telehealth platforms to deliver remote patient care effectively.
• Background in specialties such as gastroenterology, geriatrics, palliative medicine including hospice care or occupational health is advantageous.

Pay: $126,147.79 - $151,919.92 per year
