Family Support Specialist

BA in Human Services or Equivalent (credit hours in Social and Family Work)

Family Specialist Credential Level 5 from Gateways

Spanish Speaking

Case manager will-

Conduct home visits a minimum twice a year per family

Complete an IFSP ( individual family support plan-individual family goals) annually

Complete written community partnerships

Meet individually with each family at minimum once a month

Hold monthly group session/meeting with focus on child development, parenting, etc. for parents

Job Type: Full-time

Pay: $40,000.00 - $46,000.00 per year
