- Responsible for organizing, coordinating, and monitoring the non- clinical operations of Family Support Team (FST).

Support to staff, including monitoring and coordinating daily team schedules and supporting staff in both the office and field.

Serve as a liaison between patients and staff, including attending to the needs of office walk-ins and calls from participants and natural supports.

Actively participate in the team meetings two times weekly.

Triage calls, coordinate community between patients, family, other service providers and staff.

Manage and track screening and referral log for potential patients.

Assist with completing and monitoring program reports and performance outcomes.
- Maintain the Admission and Discharge Log for program to be used for required reports, data, and outcomes.
- Maintain accounting and budget records for patients and program enhancement funds.

Experience in a health care or social services setting preferred.

Ability to pass a level II Background clearance and drug test.
